If your child has multiple chronic conditions or special health care needs, there is a special program for Keystone First – CHIP Enrollees you can join a to help your child manage their conditions or needs.

What can the Complex Care Management program do for me?

The Complex Care Management program can help you:

• Understand and manage your child’s chronic health conditions.
• Identify your child’s medical and non-medical needs.
• Understand your child’s medications.
• Set goals to improve your child’s health.
• Find the right doctors and specialists to manage your child’s care.
• Connect to available resources in your community.

If you are a part of but no longer want to be in the Complex Care Management program, call 1-844-377-2447 (TTY 711). Leaving the program will not change your child’s benefits. It will also not change the way Keystone First - CHIP or the Department of Human Services (DHS) treats you or your child.
